Internship Agreement Requirements

A prototype internship agreement must be submitted and approved for all courses conducted outside of the school's approved quarters.  The agreement is submitted with the curriculum application of the program containing the internship. 

The following checklist identifies the items specifically required in the internship agreement.  Any missing, vague, or unidentified items will result in a curriculum or course disapproval until such items are corrected.

  1. Specific name and location of school
  2. Specific name and location of the internship site
  3. Exact dates or period of the internship experience
  4. Number of students to be supervised during one internship period
  5. Who is responsible for taking attendance at the internship site
  6. How often the school's program coordinator will visit the internship site
  7. Who is responsible if a student is injured at the internship site
  8. Specific conditions and procedures for terminating an intern
  9. How much notice must be given by either party to terminate the agreement (contract)
  10. The school is responsible for providing all instruction
  11. The school is responsible for the conduct of the interns at the internship site
  12. Interns will be supervised at all times during the internship
  13. The internship site will use the performance evaluation tool provided by the school
  14. The school agrees to maintain sufficient internship sites to accommodate all qualified students
  15. Student interns will not displace or replace regular employees

Note:  If a curriculum includes an internship, this must be stated on the approved enrollment agreement.
