BUREAU OF=20 PROPRIETARY SCHOOL SUPERVISION

Instructions=20 for Completing an "Application for Approval ‑ Quarters,"=20

The=20 "Application for Approval ‑ Quarters," BPSS‑6, and required = supporting=20 documentation must be submitted in the following=20 instances:

1. =20 when an=20 existing school moves;

2. =20 when=20 existing school quarters are renovated, expanded or=20 reduced;

3. =20 when=20 adding a new type of curriculum to the existing programs offered at a = school=20 (i.e., adding health curricula to a previous non‑health trade = school)=20 and;

4. =20 when the=20 ownership of a school is being transferred.

A school=20 must have written quarters approval from the Bureau of Proprietary = School=20 Supervision prior to offering any instruction. Failure to do so is a = violation of=20 Education Law and the Regulations of the Commissioner of Education and = may=20 result in disciplinary action.

The=20 "Application for Approval ‑ Quarters," BPSS‑6, must be = completed in full and=20 sent with the documentation listed below.

I. =20 Basic Application Information, BPSS‑6

School=20 Name

This is=20 the approved name listed on your license or registration or, in the case = of a=20 new school, the approved name your school will be = using.

Address

This is=20 the current address of the school. =20 In the case of a new school, this may be

the=20 mailing address for the initial contact person.

Location=20 of these Quarters

This is=20 the address where the school will operate once approval is=20 granted.

Floor=20 Level

List the=20 floor(s) on which the school will operate.

Room=20 Number (from floor plan) =

Number all=20 of the rooms, which your school will occupy, on the floor plan. Then list those same numbers = in the left=20 column.

Proposed=20 Use of Room

List how=20 you will use each particular room listed in the left hand column (i.e.,=20 reception area, student lounge, theory classroom, clinic, library,=20 etc.)

Floor=20 Space in Square Feet

List the=20 square footage of each numbered room.

Approval=20 requested by =

Enter your=20 name on this line.

Title=20

List your=20 title (i.e., director, president, owner, etc.)

Date=20

List the=20 date when the completed application is sent to the State Education=20 Department.

II. =20 Supporting Documentation Required

Floor=20 Plan

Floor=20 plans must be drawn to scale and the scale must be indicated on the=20 drawing. Each room must = be assigned=20 a number and the proposed use of the room must be indicated, for = example,=20 "classroom", "shop" "computer lab," etc. =20 The floor plans must show:

1. =20 Dimensions=20 of each room or unit of space;

2. =20 Entrances=20 and exits, stairways, corridors, fire escapes;

3. =20 Windows;

4. =20 Student=20 stations, fixed equipment and machines, furniture including student = desks and=20 chairs;

5. =20 Utilities=20 ‑ electrical outlets, sinks, etc.

Certificate=20 of Occupancy

A=20 Certificate of Occupancy is issued by local municipalities to verify = that local=20 building codes have been met and the building is suitable for = occupancy. It further states the purpose = for which=20 the quarters are to be used (i.e., commercial, residential, factory, = school,=20 etc.).

The=20 premises, in which instruction will be offered, must be approved for use = as a=20 school by the municipality in which the school will be conducting = business. Once the premises are = approved, a=20 Certificate of Occupancy will be issued listing the school quarters as = approved=20 for instructional purposes. = Only=20 that area used for the school needs to be approved for instructional=20 purposes. Therefore, = Certificates=20 of Occupancy for multi-floor buildings often will list only the floor = that the=20 school occupies as approved for instructional purposes. This is=20 acceptable.

Department=20 of Health Approval*

The=20 premises in which instruction will be offered must be approved for = occupancy by=20 the health authority of the municipality in which the school will = operate. The address listed on the = approval must=20 be the same address as the one listed on the Application for Approval - = Quarters=20 (BPSS-6). Schools outside = New York City = must arrange=20 with their local Department of Health for the inspection and forward the = letter=20 of approval with the Application for Approval - Quarters=20 (BPSS-6).

Fire=20 Approval*

The=20 premises in which instruction will be offered must be approved for = occupancy by=20 the fire authority of the municipality in which the school will = operate. The address listed on the = approval must=20 be the same address as the one listed on the Application for Approval - = Quarters=20 (BPSS-6). Schools outside = New York City = must arrange=20 with their local Department of Health for the inspection and forward the = letter=20 of approval with the Application for Approval - Quarters=20 (BPSS-6).

* =20 If the=20 school is or will be located in one of the boroughs of New York City, = the Bureau=20 of

Proprietary=20 School Supervision will arrange for the fire inspection once the = Application for=20 Approval ‑ Quarters (BPSS-6), the floor plans, and the Certificate = of Occupancy=20 are received. No separate = on-site=20 health inspection is necessary unless the school has a cafeteria or = provides=20 instruction in food service or food preparation.

Send two=20 copies of the completed application, the floor plans, the Certificate of = Occupancy and, if the school will be located outside of New York City, = the fire=20 and health approval letters to:
