Subject: Identifying Consumers as Significantly Disabled for NISH Contract

Recently, ACCES-VR was advised that consumers who receive services and are employed under a NISH (formerly the National Industries for the Severely Handicapped) or NYSID (New York State Industries for the Disabled) contract must be identified as significantly disabled by ACCES-VR. In order to comply with this requirement, ACCES-VR is implementing the following procedure effective 3/19/01.

Referral to a Community Rehabilitation Facility

When a consumer is referred to a Community Rehabilitation Facility identify the consumer as Not Significantly or Significantly Disabled/Most Significantly Disabled in the referral letter, on a form template or on the form (VES-415) for Supported Employment consumers. The referral must also cite the decision-making process, i.e. SSI, SSDB, or one or more functional limitation requiring multiple services over an extended period of time (6 months) for those that are Significantly Disabled/Most Significantly Disabled.

In District Offices operating under the CaMS system, the staff member should finalize a Referral Case Note documenting significance of disability under the "Significantly Disabled" Header. Staff should assemble the Referral Form to make the referral to the Community Rehabilitation Facility. The Referral Form will contain the Significantly Disabled Header as well as the associated text. This will provide documentation for the facility in addressing this need.
