Apply for Services at several different locations in the Franklin, Clinton, Essex and St. Lawrence Counties

Vocational rehabilitation is a process that involves an application for services, eligibility determination, vocational counseling and the development of an Individualized Plan for Employment (IPE) to provide services leading to employment. Attending an orientation session is usually the first step in the application process.

Information sessions are open to any and all people who wish to attend. Applications will be distributed to those in attendance.

If you are unable to attend one of the information sessions, please call the ACCES-VR office at 1-800-882-2803 to make other arrangements.

During the group orientation, we will give you an overview of the ACCES-VR program, which will cover such topics as:

  • The Mission of ACCES-VR
  • How one becomes eligible for services
  • ACCES-VR's goal of employment
  • Medical assessments
  • Confidential Health Questionnaire
  • Types of services Available
  • Economic need
  • Consumer rights and responsibilities
  • The Client Assistance Program (CAP)
  • The appeal process
  • Next steps in the rehabilitation process

After the group session of the orientation is completed, if you are interested in following through with your application, you will be given forms necessary to apply for VR services. You can complete these forms at that time or take them with you and mail them in later. Assistance will be provided if you have questions or concerns regarding the paperwork. In preparation for your first appointment with your VR Counselor, it would be advisable to get any medical information documenting your disability. If you are unable to get the medical information, we may be able to request it for you to determine eligibility. Once we have registered your application, a VR Counselor will contact you for an initial appointment.

High School Students

Every high school in the four county area has an assigned Vocational Rehabilitation Counselor who receives referral of high school students nearing graduation. The liaison at the high school, usually the Transition Linkage Coordinator, with the student’s permission sends the VR counselor information about the student via the Transition Referral Transmittal Sheet. Appointments are arranged for the student to meet the VR counselor before graduation to begin vocational planning. If you have a disability and are not sure that your school will refer you, you can ask your guidance counselor to refer you.
