Performance Review Protocols Overview

  1. Reviews are to be conducted a minimum of twice a year.
  2. Reviews should be conducted by the District Office Manager or their designee.
  3. Reviews are to be documented using the UCS Performance Review Form.
  4. Data on utilization of the contract and the Performance Indicators will be reviewed with the provider at the time of the review.
  5. Issues of quality and appropriateness of service should be addressed at the review and documented on the Performance Review Form.
  6. District Offices will coordinate reviews of contracts that are shared by multiple offices.
  7. Copies of the Performance Review Forms should be provided to the contractor and the original document should be placed in the District Office contract file.
  8. UCS Performance Review Forms will be signed by appropriate personnel for both ACCES-VR and the Provider.
