Performance Review Protocols Overview
- Reviews are to be conducted a minimum of twice a year.
- Reviews should be conducted by the District Office Manager or their designee.
- Reviews are to be documented using the UCS Performance Review Form.
- Data on utilization of the contract and the Performance Indicators will be reviewed with the provider at the time of the review.
- Issues of quality and appropriateness of service should be addressed at the review and documented on the Performance Review Form.
- District Offices will coordinate reviews of contracts that are shared by multiple offices.
- Copies of the Performance Review Forms should be provided to the contractor and the original document should be placed in the District Office contract file.
- UCS Performance Review Forms will be signed by appropriate personnel for both ACCES-VR and the Provider.
